DESCRIPTION:Community Health Assessment Resource Team\nOur mission is to assess the current health status of Henry County, including needs and services, and plan for implementation and utilization of services to meet those assessed needs.\n \n                                            1St Annual Fundraising Golf Tournament \n                                    (100% of funds raised go back to Henry County Schools)\n \n \nTo Whom It May Concern,\n \nHenry County C.H.A.R.T. is hosting its 1st Annual Healthy Communities Golf Tournament this Spring (June 17 @ Meadow lake Golf Club Clinton MO).  We are looking for sponsors to help reach our event goal of $30,000, which will be used to assist Henry County Schools by paying off students’ outstanding lunch account balances and purchasing supplies.  For the current school year, those expenses exceed $30,000, so your donation will significantly help our community by allowing lower/middle income families and all eight school districts in our local area to worry a little less about meeting these needs.  All donations are welcomed and appreciated.  For any donation over $150.00, you or your organization will be noted on a hole sponsorship sign and receive recognition at the tournament and on social media pages. Further donation levels are listed below.\n \nC.H.A.R.T. is a community coalition that utilizes resources from local organizations and faith-based entities to meet the needs of our tight-knit, rural community.  One of the needs identified in our most recent Community Health Needs Assessment is food insecurity.  It is our coalition’s desire to host our Annual Healthy Communities Golf Tournament to raise funds for each year’s chosen charitable effort.  This year, the coalition wishes to donate those proceeds to our schools towards those outstanding lunch balances and especially to our smaller schools that purchase much-needed clothing and shoes for students in Henry County, Missouri.\n \nIf you have any questions, please do not hesitate to contact me.
